First appearing as Cynthia Hooper in season 4, episode 2, "Something New," Mariska Hargitay Starting a relationship with Dr. Mark Greene (Anthony Edwards), Cynthia is a new desk secretary at County GenEral Their relationship is a passionate maelstrom in which neither party is certain about what it is they want. This becomes embarrassing when Mark has to go back home to see to his sick mother in San Diego and finds Cynthia tagging along, misinterpreted their relationship. They break up when Mark tells Cynthia he is not in love with her.
Mark finds Cynthia leaving the hospital when he gets back to Chicago; she shuts him off pERmanently when he attempts to get hER back. Early seasons of ER feature Mark's personal problems as a major plotline, and Cynthia shows how his carelessness with women has resulted in him unable to have a true, long-lasting relationship. Cynthia can come off as clinging or irritable in the wrong hands, but Hargitay's kind approach makes it very evident that Marks is in the wrong hands. Although Cynthia's decision to travel to San Diego without informing Mark was audacious, everything she and the audience had seen suggested that he was searching for something meaningful, and presumably considering her last-ditch attempt to mend their relationship.

Though viewers might not remember, Mariska Hargitay starred in ER prior to her Law & Order: Svu stint that defines her career. Over 15 seasons of ER, Cook County General Hospital in Chicago, Illinois has had many major, supporting, recurring, and guest characters filling the ranks of the long-running medical drama. "So at last I get an audition for ER, and I prepare quite hard for it. When I arrive, six blondes occupy the room. Like gorgeous blondes. And so I go, 'Oh wow. Well, I suppose this is not going to happen. And it relieved all the burden on me. And I went in this interview, had a great time on the audition, and was quite ready—not dishonest. But you know, you do your stuff and then you strike the flow. You then are riding the wave and you are in the flow. I thus left the audition and felt as high as a kite. Knowing that's unusual when we actually leave an audition feeling, "I killed it,'" made me very glad.

Learning she didn't have the part, Hargitay's intuition was right. Later that day, though, she went through another audition for John Levey, the casting director. She turned to Levey and inquired,
"'But why did I not get ER? This is really absurd. Let's head straight to John Wells' [the executive producer of ER] office right now. You were really great, he said. [But] I see this happening for this part. I also prefer to cast nearer to type. And the girl was a somewhat delicate, frail, fractured, worried one. And you really are really robust. You not precisely fit the kind.'" Hargitay responded, "'Hold on. She did not like that justification. I'm not understanding you since I'm not that way in real life and you're telling me I acted better than anybody? and he goes, "Yeah." And by the time I arrived home, they had phoned and extended an offer for the part." Olivia Benson, Hargitay's hard-nosed character on Law & Order: SVU, is far more in line with this tenacity than her soft-spoken one on ER. That same precise intensity probably landed her both parts, but for quite different reasons. Though Olivia is far more competent than the flighty Cynthia, both Olivia and Cynthia have a special talent for managing demanding situations. But Mariska Hargitay has been able to play Olivia for so long because her character is unusual and deep; few on television combine persistence with empathy.
