This might seem obvious for those aware of the backstory for Maria, but for viewers who hadn't examined this fact:  Maria was born from James' idealized wishes for Mary; an idealized representation, highlighting James' repressed desires which might’ve not been recognized, a response driven by his inability to accept what actually occurred. Maria looks, sounds like Mary—but sexier; this further adds meaning.

Mary's illness begins with a cough—indicated via her VHS tape.  This implies Maria, as an echo of Mary, may have the same illness and eventually die from it. A potentially incredibly bleak possibility – particularly as her entire creation comes from James replicating and focusing entirely on the mental and emotional image of Mary. The resulting reality would end with Maria contracting the very same disease.

The implications?  Maria's cough is deeply relevant! It possibly further develops a fan theory that James is trapped; his actions resulting in perpetual torment which makes some parts rather creepy and extremely unsettling for viewers. This concept would explain all those other James bodies around Silent Hill—a neverending nightmare!

Thankfully, you don’t need a New Game Plus to get this one; although you do need quite a bit of specific, detailed actions and events for a certain result to occur, to completely meet certain conditions in which this outcome would eventually occur.

  • Ignore Mary's letter and photo. Ignore everything Mary.
  • Maintain James' health— heal whenever you take damage.
  • Avoid examining personal items during that search and gameplay activities.
  • Use the Woman coin to complete the Coin Box Puzzle.
  • Protect Maria during that journey.
  • Use that Ripe Apple in the mirror puzzle and then exit Pyramid Head's fight using that red door.

(Bloober Team changed Maria's look in the remake; this reduces some symbolism, this becomes particularly notable concerning how her appearance is supposed to add greater symbolism that’s slightly removed now!)
